Get Ready: An Introduction to CLTS   offers an introduction to Wisconsin’s Children’s Long Term Support (CLTS) Program. It has ideas to prepare your family to get the most out of this valuable program.
We’ll cover:
• How the CLTS Program supports kids & families, and examples of what may be covered
• Eligibility and enrollment, and tips for being ready for the screening process
• Understanding your child’s Individual Service Plan (ISP)
• The role of the Support and Service Coordinator
• Lessons from other families

What's After High School? offers families a broad overview of the transition to adult life for young adults with disabilities and special health care needs. The event is most helpful for families with children ages 14 - 21, and the educators and other providers who support them.

Youth health transition

Introduction to Youth Health Transition
The Process of Getting Ready for Health Care as an Adult introduces the steps of moving from pediatric to adult health care.

Build Your Bridge
Moving from Child to Adult Health Care introduces and discusses when to start thinking about health care transition to adult care providers as well as offers helpful tools and resources.

Dreaming Differently
This resource is available to meet the needs of a unique audience with the Build Your Bridge curriculum adapted for families raising children with significant intellectual or developmental disabilities and medical complexity.

Bridging the Gap
This resource is an overview of youth health transition with information tailored to different, mixed audiences like parents, teachers, nurses, and others.

What’s After High School
This resource is a broad overview of the transition to adult life for youth with disabilities and special health care needs and includes envisioning a youth’s future, post-secondary concerns, long-term supports, transitioning to adult health care, legal considerations, and supported decision making.

is a medical home series for families. The presentation and discussion seriesincludes:

Caring for the Whole Family
This is an opportunity to reflect on the experience of learning that your child is developing differently. Resources are explored in a supportive environment.

Assembling a Care Notebook
This presentation shows instructions on how to create a ready resource of pertinent information for your child and caretakers.

Requesting a Shared Plan of Care
This presentation shows how to work with your child’s medical provider to develop a shared plan of care which includes a medical summary, a family’s strengths, and goals.

Exploring Care Mapping
This shows how to design a care map for your family that will serve to show others at glace all of the people and resources supporting your child’s development.

This presentation is an overview of private and public health insurance benefits, community supports, and services and other resources for children with special health needs and or disabilities.
